Who is it for?
Hospital at Home gives one-to-one care in a patient's own home,
providing intensive support and treatment that would traditionally
have been provided in a hospital setting. It
incorporates services that were previously provided by
the Rapid Response Service.
Hospital at Home aims to:
- avoid admission to hospital - for example by providing a
proactive approach, including social work support, to help prevent
the need for hospital admission
- minimise the amount of time spent in hospital. Early, yet safe,
discharge from hospital can be made a reality by putting in a
package of healthcare and social care support which can be
delivered in the patient's own home
- free up valuable hospital resources and beds using the above
approaches
- satisfy, where appropriate, increasing patient preference for
treatment at home
- support quicker recovery - evidence suggests that patients
who are cared for in the comfort of their own home can benefit from
a speedier recovery.
Once a patient is fit and well they are discharged from the
service, just as they would be from hospital.
Would I be eligible?
Hospital at Home is available to anyone over the age of 18 whose
primary problem is not a mental health issue. There are situations
where referral to the service would not be appropriate, such as
where there is an acute change in a patient's condition, or where
treatment can only be given in hospital. This can be discussed
during an assessment of your condition.
How much does it cost?
There is no charge for the Hospital at Home service.
