Intermediate Care services can help to reduce unnecessary admissions to hospital or premature admission to long term residential care.
Intermediate Care services aim to support adults to regain their independence after a short stay in hospital.
If you have been in hospital, perhaps because you are unwell or have had an operation, you may need some temporary help when it is time to return home.
Hospital at Home gives one-to-one care in a patient’s own home, providing intensive support and treatment that would traditionally have been provided in a hospital setting.
The Early Intervention Service will look at your ability to carry out everyday tasks and, where suitable, design activity programmes to improve your ability and maintain or regain your independence.
